Summary

Project aimed to develop an integrated unit and seamless transition for young people with disabilities, moving from childrens’ into adults’ service provision.

Description

The project created a new role of project co-ordinator, in order to bring together a range of agencies to work together in new and integrated ways both across services and across the child/ adult service divide. The role also had responsibility for identifying and developing new systems that would support integrated working and ensure a more effective service based on identifying and tracking the needs of individual children, so that they would not "slip through the net".

Outcomes

  • A range of agencies including health, social care and Connexions developed greater awareness of each other’s role and responsibilities
  • Parents and carers felt reassured and supported
  • A data base was created which has helped underpin better services tailored to the individual need so of the child, by mapping provision and tracking the child’s progress
  • Improvements in external inspection reports

Themes and Issues

  • Difficulties in amalgamating or integrating "paperwork" across different agencies
  • Difficulties in truly integrating service across children’s and adult services within the Local Authority
  • Increasing separation between the social work role for children and for adults
  • Transitional, "start up" roles require different skills and expertise than those needed to embed a service.
